Following [livejournal.com profile] _alick_'s excellent blog, I discovered the site of Etiquette & Protocol. After some casual browsing I came upon this awkward piece of etiquette:
When you meet someone who is unable to shake hands, either due to the lack of a functioning arm or hand, or for some other medical reason, it is still considered appropriate and polite to extend your own right hand in greeting....
Now this is something that I have real trouble following. If my disabled interlocutor doesn't extend something first, I can only anticipate further embarassement for both of us if I take the initiative.

Upd: This one about names is awesome!
